Under 18's Parent / Guardian Declaration: I confirm that I am the parent or legal guardian of the applicant. I understand the risks involved in motorsport and give my consent for my child to participate in Stoxkarts events. I confirm that I have read and agree to the Stoxkarts Rules and Regulations and accept responsibility for my child’s conduct and safety at all Stoxkarts events and activities.

MEDICAL DECLARATION (ALL DRIVERS) You must answer all medical questions truthfully and to the best of your knowledge and belief.

These questions are used by Stoxkarts Ltd. to assess your suitability to compete in motorsport.

If you answer YES to any question, except Question 6 & 7 you may be required to provide a doctor’s letter before a licence is issued.

Driver Declaration

I confirm that I have read, understood and agree to abide by the Stoxkarts Rules and Regulations.

I understand that motorsport is a high-risk, contact sport and that I participate entirely at my own risk. I accept that Stoxkarts Ltd, its officials, promoters and volunteers do not provide personal injury insurance for competitors and shall not be liable for any loss, injury or damage arising from my participation, except where required by law.

I agree to conduct myself in a safe and responsible manner at all times and accept responsibility for my own actions and the actions of my pit crew, mechanics and family members.

I consent to Stoxkarts Ltd using my name, race number, photographs and video footage for promotional and media purposes.

I understand that submission of this application does not guarantee the issue of a licence and that Stoxkarts Ltd reserves the right to refuse, suspend or withdraw a licence where necessary in the interests of safety, safeguarding, governance or the good order of the sport.
