You keep this form, DO NOT include it with your licence.
To Download and Print Simply Click on the Yellow Text in each section.
To be Completed by all drivers both Junior and Senior.
To be Completed by all drivers racing for 1 day only
Parent/Guardian to sign for any Driver over 16 but Under 18 years of age.
All Registered Drivers need to complete this form and wear it at all meetings. DO NOT Return it with your Licence.
To be completed by all drivers including hire drivers. Must be returned with your full licence application form.

You need to complete the Digital Stoxkarts Licence form and declaration.
Day Licence/ Hire Driver
You need to download, print and complete Form SX/3 Stoxkarts Day Licence and SX/7 Flags. You need to bring these with you to your first meeting.
Day Licence/ Hire Driver UNDER 18.
You need to download, print and complete Form SX/3 Stoxkarts Day Licence, SX/4 Parent Guardian agreement and SX/7 Flags. You need to bring these with you on the day.
